Initiates, develops, implements and makes necessary revisions to the plan of care in collaboration with the physician and other health care professional's involved in care. Stays informed about changes in the field of nursing and home health care; shares information with appropriate organization personnel. Provides help in assessment, planning, implementation and evaluation of patient and family/caregiver care to all clinical personnel as indicated.
Observes and reports the patient's response to treatment and any changes in his/her condition to the supervising nurse and/ician. Treats patients to relieve pain, develop and restore function, and / or maintain maximum functioning using acceptable standards of practice. Instructs and informs participating members of the health care team, the patient, and the family/caregivers regarding the plan of care, functional limitations, and progress toward goals. The Director of Nursing must be a Registered Nurse who is currently licensed to practice in the state of Louisiana and has at least 3 years of experience as a Registered Nurse. One of these years must consist of full-time experience in providing direct patient care in a home health setting.
